Receipts for outpatient consultations or tests related to this treatment, if applicable to your policy.
An obstetric history note (Gravida/Para/Living children/Abortions) from the treating doctor - part of the hospital's own Claim Form Part B. Pre-natal and post-natal records are usually needed too, and post-natal ones are normally fine to submit after the main claim, since that care often isn't finished yet when you file.
